India’s New Transgender Rules Disrupt Healthcare, Raise Concerns Among Doctors

India’s revised transgender recognition law has disrupted access to gender-affirming healthcare, leaving many transgender people unable to continue hormone therapy or proceed with planned surgeries. The law, introduced in March 2026, replaces self-identification with a requirement for certification by a medical panel before legal gender recognition. However, the government has not clarified how these panels will function, prompting several clinics to pause treatments over legal uncertainty.

Doctors and activists warn that the new rules have created confusion and fear among healthcare providers, with concerns that offering hormone therapy or surgeries could expose them to legal action. Several transgender individuals across cities such as Hyderabad, Bengaluru, and Kozhikode told Reuters their treatment plans had been delayed or halted. Medical experts caution that abruptly stopping hormone replacement therapy can have serious health consequences, while activists fear many people may turn to unregulated providers.

The government says the changes are intended to prevent misuse of welfare schemes, strengthen safeguards, and curb exploitation. However, critics argue the law weakens the Supreme Court’s landmark 2014 recognition of transgender rights and could further marginalize an already vulnerable community. Activists are urging the government to provide clear implementation guidelines to ensure uninterrupted access to essential healthcare while protecting the rights and dignity of transgender people.
